The folder below contains the Common Feedback Form on Drive. We have also developed:
- Guidelines for the use of the form
- Instructions for using the template version of the form in Survey Gizmo.
- A similar form that can be used for conferences rather than professional development events

Survey Gizmo
Survey Gizmo has an extensive online help section – to access it, log in with your department’s login and password, and select the “Need Help?” button at the top right. This will bring you to a page with links to a lot of different resources and a way to search for help with specific questions.
If you’re having trouble logging in, or if you need more guidance than the website can give you, submit a tech ticket and it will be routed to a member of the R&E department. We have also written up some guidelines, below, that might be helpful, although Survey Gizmo changes their interface frequently so some of the details might now be obsolete.
